If you have any scheduled actions that are due to be taken, (and your smartphone’s native setting is turned “on” for badges), the Care at Home icon on your smartphone will have “badge” with a number inside of it, indicating the number of currently scheduled measures that are due to be recorded. If you don’t see a badge, you can still record measures. You can record measures at any time.
To record your measures, from your Care at Home home screen,

Tap on the “Check In” icon in the upper right area of your screen to open your Check In screen. (Again, if you have any scheduled measures due, the Check In icon will be badged.)

To respond to symptom- or condition- related questions, tap on the dark grey check mark to open the series of questions in your Care Template. You will see a list of measures.

Tap on the down arrow for the first measure – read and answer the question presented.

After responding to the first question, additional conditional question(s) on the same measure may be presented, which will need to be answered. You may, for some measures, also be presented with a concern question, which you need to respond to as well. (NOTE: if you are NOT experiencing one or more of the symptoms on the list of symptom measures presented to you as a scheduled action in your Care Template, you may skip that symptom, and your response to the question for that symptom will be recorded as “None” in your Care at Home app, and “None” in the report reviewed by your Care at Home Provider.)

When the question (or questions) for that measure are completed, continue your responses by tapping on the down arrow for next measure.

Follow this process until you have completed all of the questions presented. Note you may have to scroll down to reveal some questions at the bottom of the screen.

Once your responses for all of the questions are completed tap on “Save” in the upper right corner of the screen. You will then be returned to your Check In screen.
Back on your Check In screen, if you do not have any other pending actions, tap “Submit” in the upper right corner of the screen. However, if you have other elements, that is, additional measures to record, for example, vital signs, (e.g., blood pressure, heart rate, etc.) for your Care Template to complete, follow the guidance below to record those measures:

Please record any and all remaining measures for any vitals or other remaining elements (e.g., sleep or steps) in your Care Template.

Manual data entry: Simply add your data to the text box for that measure.

Machine Entry of data: If you have connected a third-party device, sensor or wearable to your Care at Home app, you will see a “Sync” button. When you tap that button, the recently recorded data from your device, sensor or wearable will be automatically “machine entered” into your Care at Home app. Some Care Templates require machine entry. If you do not see a text box along with the Sync button, that measure requires machine entry, and you will not be able to manually add your data.

When you have completed all elements of your Care Template, tap on “Submit” in the upper right corner of the screen.

Note: If you do not have any additional measures after completing your symptom- or condition- related questions, you can simply tap the “Submit” button after you have Saved your responses.
Also note that some Care Templates may not have any symptom- or condition- related questions, and you will only have vitals or wellness measures to complete. In this case, simply record your vital or wellness measures and tap “Submit” when you have completed those measures.
Once you have submitted, your responses will be processed and recorded for your Care at Home account. If you are in a Directed Care Program with a Care at Home Provider, your responses will be monitored and accessible by your Care at Home Provider team.
