If you are a patient’s Proxy participating in a Provider’s Directed Care Program on behalf of a patient (a “Dependent”), you can access your loved one’s Dependent profile from your own Care at Home account. To do so:
-
Log in to your Care at Home account.
-
If not already expanded, tap “expand” in the Circle of Care header on your home screen to review the Dependent account that you’ve been authorized to support. (Note: If other people, in addition to your Dependent, have invited you to support them using Care at Home, their names will also be listed in your Circle of Care.)
-
Tap in the name panel of the Dependent you are supporting to access their profile. (If you simply want to record actions on behalf of the Dependent, from the expanded Circle of Care section of your home screen, simply tap on the “Check In” icon on the right side of the name panel and go directly to the Dependent’s care plan to record their actions.)
Set up “Caregiver View” to access your Dependent’s Profile: You can also set your home screen to more quickly view the Dependent’s account. To do so:
-
Log in to your Care at Home account
-
Tap on “Member View” in the upper right corner of your home screen. This will reset the default view to “Caregiver View”. From this view you can more immediately access your loved one’s Dependent profile to view and manage their account. The Caregiver View will now be the default view for your Care at Home account upon future log ins.
You can toggle back and forth to reset the default view between Member View and Caregiver View at any time to support your most convenient use of the Care at Home app.
