Independent Care Plan for Consumers

Managing regimen elements in your Care Plan

Whether you are a Patient in a Provider’s Directed Care Program or an independent consumer, the first two steps in the process for accessing your Care Plan to manage your regimen elements, which you independently use for your own self-care, is the same:

  1. Login to your Care at Home app.

  2. At the bottom of your home screen you will see a few icons, including “Plan”. Tap on the Plan icon to open the Care Plan Screen.

    1. If you are a patient in a Provider’s Directed Care Program, you will see elements of your provisioned Care Template listed.

    2. If you are a patient in a Provider’s Directed Care Program, and you have previously independently added your own care plan elements, you will see both the elements of your provisioned Care Template from your Care at Home Provider, and the elements of your independently created care plan listed.

    3. If you are a consumer, who has previously created care plan elements, you will see those elements of your independently created care plan.

    4. If you are a consumer who is new to Care at Home, the screen will not have any entries. To learn how to Create/Add your own independently created care plan elements, please search the Knowledge base for the following: “Creating/Adding your own Care Plan”.

To MODIFY elements of your independently created care plan: You may only edit or modify the elements of your care plan that you have independently established for your own self-care. To modify:

After tapping on the Plan icon, tap on the name panel of any element listed to open that element of your care plan.

  1. For Vitals and Wellness, a “Summary” screen will be presented. If “Created by” states “(Your Name) / Self”, you can fully modify this care plan element. Tap on the Vital or Wellness measure to begin the editing process.

    1. Tap on “Edit” at the top right of the Summary screen. On the next screen, tap on the arrow to the right of the element selected to open the Details screen and make your modifications. For each change be sure to tap “Save” in the upper right corner of each screen.

    2. When all modifications have been made tap “Save” in the upper right corner of the Details screen.

    3. Next, back on the Summary screen, you can modify your Reminders and set Alert notifications to your Network – your Circle of Care – for the symptom selected. (See Modifying Reminders and Alerts for all elements of your Care Plan below.)

  2. For Symptom regimen, tap on the panel under the heading “Symptom” to open the measure you want to modify (the specific name will be under “Symptom” in the name panel). (NOTE: you may have more than one symptom listed, in which case, each symptom will need to be modified independently.)

    1. At the bottom of the next screen, if “Created by” states “(Your Name) / Self”, you can modify this symptom measure.

    2. Tap on “Edit” at the top right of the Summary screen to open edit access to the select symptom.

    3. Next, tap on the arrow to the right of the symptom name to open the Details screen.

    4. Select the arrow at the right of the panel which you want to modify (“Symptom Alert Rule”, “When you will do this”, “At what times of the day”, and “How long you will need to do this”). For each element of the symptom that you modify, be sure to tap “Save” in the upper right of the screen.

    5. After you have saved each element of the Symptom that you have modified, you will be returned to the Details screen where you can initiate additional modifications of the selected symptom.

    6. When you have made all of your modifications, tap “Save” at the top right of the Details screen.

    7. Next, back on the Summary screen, you can modify your Reminders and set Alert notifications to your Network – your Circle of Care for the symptom selected. (See Modifying Reminders and Alerts for all elements of your Care Plan below.)

You will need to follow this process for each individual symptom you wish to modify.

3. To modify Medications: After tapping on the Plan icon, tap on the name panel of any Medication listed to open that element of your care plan’s “Medication Summary”. If you have independently added the Medication to your care plan, you will be able to fully modify this care plan element. Tap on the panel for the Medication to begin the editing process.

a. Tap on any one of the pencil icons to the right of any element of the medication regimen; then, make your modification. Once completed, tap Next in the lower right corner of the screen (you may need to scroll down to view the Next button.)

b. Once you have gone through all elements of your medication and completed your modifications, on the last screen tap “Save” at the bottom right of the screen (you may need to scroll down to reveal the Save button.)

After completing any modified care plan element, tap the back arrow at the top left of the screen to return to the home screen.
