Enhance your Patient Care Plan in your Provider’s Directed Care Program

Patients in a Directed Care Program will have a Care Template provisioned to their Care at Home account from their Provider. Patients can enlarge their care plan by adding additional elements (e.g., medications, supplements, and other health elements) to create a broader care plan, but are restricted in modifying the regimen elements of their Provisioned Care Template from their Provider.

A patient may not add to or delete any specific elements in the Care Template provisioned to them by the Care at Home Provider.

A patient may, however, make certain modifications to a provisioned Care Template:

Reminders – default to “on” upon the provision of your Care Template. With Reminders “on” (check marked – open circle with white center and checkmark), you will receive notifications when the individual element in the Care Template is scheduled. Changing the setting for Reminders to “off” (unchecked -dark filled circle with white checkmark) means you will not receive reminders when you have scheduled your measure to be recorded.

Alerts – default to “off” to your Circle of Care/Network upon the provision of your Care Template. With Alerts, you can let your Circle of Care/Network (others who are supporting you using their own Care at Home app) know that you have missed a scheduled recording of the individual element in your Care Template. To set Alerts for your Circle of Care, tap on the arrow at the right of the Alerts panel, make your selection, then tap “Save” in the upper right corner of the screen. Note: Reminders must be set to “on” to enable Alerts to your Circle of Care.

Modifying regimen elements in your Provisioned Care Plan from your Care at Home Provider

Login to your Care at Home app.

At the bottom of your home screen you will see a few icons, including “Plan”. Tap on the Plan icon to open the Care Plan Screen. (As a participant in your Provider’s Directed Care Program, you will see elements of your provisioned Care Plan listed.)
To MODIFY certain elements of your Provisioned Care Plan:

After tapping on the Plan icon, tap on the name panel of any element listed to open that element of your care plan.

For elements under the Vitals and Wellness heading, after tapping on the specific vital or wellness measure, a “Summary” screen will be presented. Tap on the Vital or Wellness measure to begin the editing process.

Tap on “Edit” at the top right of the Summary screen.

Next, you can modify Reminders and set Alert notifications to your Network – your Circle of Care for the symptom selected.

to modify Reminders, simply tap on the check mark from “on” (default) to “off”. Then tap “Save” in the upper right of the screen.

to modify Alerts, tap on the arrow at the right of the alerts. On the next screen make your selection and tap “Save” in the upper right of the screen.

After making edits for Reminders or Alerts, tap “Save” in the upper right corner of the Summary screen to complete your modifications.

After completing your edits, tap the back arrow at the top left of the screen to return to the home screen.

For Symptom regimen, tap on the panel under the heading “Symptom” to open the list of symptom measures associated with the selected regimen. (NOTE: you may have more than one symptom regimen, in which case, each regimen will need to be modified independently.)

Tap on the panel with the symptom measure that you wish to modify.

On the next screen, tap on “Edit” at the top right of the Summary to open edit access to the select symptom.

Next, you can modify Reminders and set Alert notifications to your Network – your Circle of Care for the symptom selected.

to modify Alerts, tap on the arrow at the right of the alerts. On the next screen make your selection and tap “Save” in the upper right of the screen.

Follow this process for each symptom in the selected regimen, tapping “Save” in the upper right of the screen as you complete each symptom modification in the regimen.

After making edits for Reminders or Alerts, tap “Save” in the upper right corner of the Summary screen to complete your modifications.

After completing your edits, tap the back arrow at the top left of the screen to return to the home screen.

NOTE: If any Medication is made part of your provisioned Care Template from your Care at Home Provider, you will not be able to modify this Care Template element.
