For Patients who wish to participate in a Directed Care Program with a Care at Home Provider:

Your Care at Home Provider will ask you about participating in a Connected Care Program for Directed Care.

Upon acknowledging your wish to participate, your Care at Home Provider will send you an invitation via email to enroll and register in the Program.

You will need to follow the guidance in the email invitation to download and install Care at Home from Lifeguard™ to add the mobile application to your iPhone or Android Smartphone.

Once installed, your Care at Home mobile app will allow you to fully participate in the Directed Care Program from your Care at Home Provider.
