A Dependent and Proxy relationship may be established by a Provider using Care at Home in a Directed Care Program. In such circumstance,

a patient authorizes the Care at Home Provider to assign, as a Proxy, a person whom the patient identifies, and who agrees to participate on behalf of that patient in the Provider’s Directed Care Program.

The Provider then invites the intended recipient as the patient’s authorized Proxy to participate in Provider’s Directed Care Program on behalf of that patient.

The invited Proxy receives a program invitation via email.

The invited Proxy, then needs to accept the invitation, download and install the Care at Home mobile app by following the guidance in the email invitation.

Once the Proxy accepts participation in the Program, the patient then becomes the Dependent of that Proxy.
The Proxy then can, using Care at Home on behalf of the Dependent, engage and communicate with the Provider’s care team as determined by the rules established by that Provider’s Directed Care Program. A Proxy may invite other family and friends to support them in supporting the Dependent as a member of the Dependent’s Circle of Care.
