Enrolling in a Care at Home Provider’s Directed Care Program after Care at Home has already been installed on a smartphone

If the Care at Home mobile app has been installed on the Patient/Proxy’s smartphone before being enrolled in a Care at Home Directed Care Program from Care at Home Provider, the patient or proxy will still need to register in the program. Following the same directions for newly invited program participants, you will need to:

1. respond to your emailed program invite from Care at Home (from sender name: “Care at Home”),

2. accept the invitation, and

3. accept the program’s Informed Consent. (The Informed Consent is from your Care at Home Provider. To complete program enrollment, you must tap “Accept” on the Informed Consent, otherwise, you will not be permitted to participate in the Provider’s Directed Care Program.)

4. Once that is done, you can simply log into your established Care at Home account and your newly provisioned care plan and other related information (if any) from your Care at Home Provider will be available in your Care at Home account.

NOTE: the email address for your previously installed application MUST BE THE SAME as the email address used to enroll you in your new Care at Home Provider’s Directed Care Program – otherwise you will be establishing a separate Care at Home Account. You can use the same email address for as many Care at Home Programs in which you may be enrolled – and access all Programs from the same Care at Home account.

If you ever need to re-install your Care at Home mobile app on your smartphone, you can do so directly from either the Apple App Store or the Google Play Store by searching for ”Care at Home from Lifeguard™” and then installing the app.

As a Care at Home Provider Directed Care Program-enrolled Patient, the re-installed Care at Home application will automatically recognize you as a member of a Provider Program, and previously recorded data will be available in your newly re-installed Care at Home mobile app.

Note: Remember you must use the same email address used for the Directed Care Program. Only then will Care at Home remember the previous account information and repopulate information previously recorded and saved in that Care at Home account.
