For Patients (or their Proxy) who are invited to enroll in a program supported by a Care at Home Provider, the invitation process will support and provide guidance on registration in the Provider’s Directed Care Program, including download and installation of the Care at Home mobile app for the Program.

Self-report or choose a Proxy: To participate in a Directed Care Program using Care at Home, the Patient must enroll with the Care at Home Provider organization. With guidance from the Care at Home Provider organization, Patients may participate directly (“self-report”) or may authorize another person as a designate (a patient “Proxy”) who can act and engage in Care at Home – and communicate with the Care at Home Provider organization’s clinical team – as an authorized “healthcare representative” (or similar term) on behalf of the Patient.

Email invitation: To participate in a Directed Care Program using Care at Home, the Patient (or Proxy) must first be invited to participate in the Program. Your Care at Home Provider will email to the Patient (or a Proxy authorized by the Patient) a program invitation from Care at Home (from sender name: “Care at Home”). (Please check your spam folders if you do not see the email in your inbox.) It is most convenient, for completing the process of registration, to view the email from the smartphone that will be used for the Directed Care Program. (NOTE: If you enroll using something other than the smartphone that you will be using to participate using Care at Home, you will have to switch to that smartphone before you can complete the full enrollment, registration and installation process of the Care at Home mobile app.)

Accept Invitation: Open your email and follow the guidance for program enrollment beginning with tapping on the Accept Invite button the body of the email. This will open a registration form which you need to complete. Once you’ve verified and entered your information, selected and confirmed a password (please write down the email address that you will be using and your password for Care at Home – as you will need it to log in – and keep in a secure location), please review the Terms of Use and tap the check box to confirm your acceptance of those Terms for using Care at Home.

Accept Informed Consent: Next, you will be presented with an Informed Consent form, which you need to read and accept. This Informed Consent is from your Care at Home Provider. To complete program enrollment, you must tap “Accept” on the Informed Consent, otherwise, you will not be permitted to participate in the Provider’s Directed Care Program.

Download and install the Care at Home mobile application: Immediately following completion of your enrollment in the program, as part of the same workflow on your smartphone, you will be presented with a screen with information on installing Care at Home. From the smartphone you will be using for participation in the Care at Home Program, select the appropriate store (App Store for iPhone; Google Play Store for Android) and tap the button to access the store location for Care at Home from Lifeguard™. Follow guidance from the store to download and install your Care at Home mobile application.

Ready for Log in: Once you have installed your Care at Home app, you will be ready to log in and participate in your Directed Care Program from your Provider. (HINT: On your Care at Home app log in screen, if you tap the “Remember me” box, your email address (only) will be saved to speed future logins.)
