Complete and verify your Care at Home Profile: Log in to your Care at Home app. Tap on your photo (or the graphic photo icon placeholder) at the top left corner of your home screen to open your Profile. Then tap on “Edit” in the name panel at the top of the Profile screen to open the profile of your Care at Home Account.

Complete all information: Enter any missing information and review any pre-populated information that may be presented and ensure that all information is correct (e.g., spelling of your first and last name, your date of birth, as well as your email address). NOTE: If you are participating in a Care at Home Provider’s Directed Care Program, please notify your Provider if you make any changes to any information, especially any pre-populated information, that you update in your account profile. It is critical that the data in your Care at Home Account profile matches your Provider’s patient data.

Mobile Phone Number: If you haven’t already done so during the initial Care at Home registration, add your mobile phone number – necessary for some in-app supported features, support calls, as well as account recovery support, if ever needed.

Your Photo or Special Image: Add or update your account profile picture to help other members to whom you authorize access to your Care at Home account, and, if you are participating in a Care at Home Provider’s Directed Care Program, your clinical team, to quickly identify you and associate you with your account. To add or change a photo, or a meaningful image, tap on “Edit image” under the current image (or the “No Image” square) at the top of the Account profile to open a pop-up window with options to “Take photo” or, to select images already captured on your smartphone, “Select from Gallery” (Android) / “Select from Photos” (iPhone). Follow the appropriate steps to take a photo or add the image you wish to select.

Your Location: Complete the rest of your account profile as necessary and make sure your profile includes your address/location information for better customer service.

When completed, tap “Save” to retain the updated information in your profile.
