Care at Home (for Patients or their Proxies) is essentially the same mobile app as Care at Home (for consumers) with one significant exception: it is offered exclusively through Care at Home Providers (healthcare providers, managed care organizations, health plans or health support services) and engages the Patient (or their authorized Proxy) in better self-care along their healthcare journey, with support from a Care at Home Provider. Your Care at Home Provider will provision a Care Template and add it to your Care at Home Account. The Care at Home mobile app enables the Care at Home Provider to remotely monitor and manage the Patient under Directed Care.
Note: some Care at Home Providers may also co-brand Care at Home uniquely for their organization, with their own logo and color scheme. If a Care at Home Provider is enabled as a custom co-brand, once you fully register in the Program, you may experience a conversion of color scheme for Care at Home in your mobile app (from consumer-based brand to a Care at Home Provider co-branded label) and see a uniquely branded app icon on your smartphone’s home screen.
