Care at Home from Lifeguard™ (Care at Home) is a free-to-consumer digital health application available in the App Store (iPhone) or Play Store (Android), designed to help anyone involved in managing their own health journey, to support their own self-Care at Home. Care at Home allows for communication between and among those people who are engaged in supporting a person’s care: a person’s family and/or friends (caregivers invited into the person’s Circle of Care) and, of course, with the individual person who is using Care at Home for their own health journey. Care at Home is for consumers who want support in managing their own self-care, or for other people who are invited into a person’s Circle of Care – to support that other person. Care at Home makes it easy to organize and access important health information, make more informed decisions, manage key tasks, and seamlessly coordinate with the others who are participating in providing support.
