To independently add Vital measure(s) to your care plan in Care at Home:
Regardless of being a patient or a consumer, the process for adding elements to your care plan is the same.

Login to your Care at Home app.

At the bottom of your home screen, you will see a few icons, including “Plan”. Tap on the Plan icon.

If you are a patient in a Provider’s Directed Care Program, and your Care at Home Provider has included vital(s) in your Care Template, you will see those regimens listed under a “Vitals” heading. Otherwise, you will not see a “Vitals” heading.

If you are a consumer using Care at Home from Lifeguard™, and have not previously added any vitals, the “Vitals” header will not appear. If you have previously independently added a vital regimen for yourself as a consumer, you will see it listed under the “Vitals” heading. )

Tap on the “+” sign at the top right of the Care Plan screen to open the Regimen screen.

On the Regimen screen, you will see an option under the “Add a Care Plan Regimen” header for “Vitals” for you to independently build and add your own self-care elements to your care plan. (Remember, if you are a patient in a Provider’s Directed Care Program, your Provider will NOT BE NOTIFIED if the new care plan elements that you choose to add have any alert notifications associated to them.)

Tap on the “Vitals” panel to open the “Choose” selection screen.

Select one vital measure by tapping the check mark to the right of the vital name, then tap “Next”

Tap the arrow on the “Acceptable Range” screen to establish the value for minimum and maximum thresholds for the selected vital sign. (Please note that there is an exception for the Weight vital measure which only establishes a schedule and does not have an associated range.) Each vital measure (except Weight) has default settings which you can change by tapping on the arrows above and below the text box(es) presented for that vital measure – or tap inside the text box to manually enter the value. After you select your range, tap on “Save”.

Next tap on the arrow under “When you will need to do this” to open the “Day(s)” screen. On the Day(s) screen, the days of the week for which you wish to schedule reminders for recording your measures defaults to Daily. If you want to modify, just tap the previously checked selection to uncheck and remove that day from your scheduled reminder for that day. You can reselect the day(s) you wish to add back your schedule reminder for that day. When completed tap “Save”. Note, if you select “As Needed” Care at Home will not generate any Reminders.

Back on the “Details” screen”, tap the arrow under the “At what times of the day” heading to access the Time(s) screen. Choose the time(s) of day when you wish to receive your reminder for the selected vital measure. Note: you can choose multiple and specific times under the “Advanced” heading. If selecting Advanced setting for time(s), tap “Save” when completed to save specific time(s). To complete time of day, tap “Save” in the upper right corner.

The last step is to select the dates under the “How long will you need to do this” heading to establish what dates you want to begin and end your scheduled recording of your vital measure. Note that the beginning date defaults to the current date. Also note that you cannot establish a beginning date before the current date, but you can schedule a beginning date in the future. For the end date, note that it defaults to “Never Expires”. To set specific dates, tap on “Starts” and “Ends” to establish specific dates for scheduling this vital measure. When completed tap “Save”.

When all selections have been made on the Details screen tap “Next” at the top right of the screen.

Next, on the Summary screen you can set your Reminders and Alerts settings.
• With Reminders “on” (default check marked), you will receive notifications when this newly added Vital measure is scheduled. Setting Reminders to “off” (unchecked) means you will not receive reminders when you are scheduled to record.
• With Alerts, you can let your Circle of Care (others who are supporting you using their own Care at Home app) know that you have missed a scheduled recording of this newly added Vital measure. Note: if you deselect Reminders and set to “off”, Alerts will be disabled for this vital measure. -
The final step is to tap “Save” on the Summary screen to complete the process.
Repeat this process for each Vital measure you wish to establish and schedule for yourself. Each individual vital measure is independently scheduled so that you can manage each uniquely according to your own needs.
IF YOU RECORD A MEASURE THAT YOU SET FOR YOUR OWN INDEPENDENT SELF-CARE, WHICH EXCEEDS A THRESHOLD, AND YOU PARTICIPATE IN A PROVIDER’S DIRECTED CARE PROGRAM, YOUR PROIVIDER WILL NOT BE NOTIFIED ABOUT THAT VITAL MEASURE, WHICH YOU SET FOR YOUR OWN INDEPENDENT SELF-CARE.
The next time you log in, your newly added Vital measures can be accessed for recording by tapping the Check in icon in the upper right corner of your home screen listed under the heading of “(your name) / Self”. If you set up a schedule for your self-managed vital measures, and selected to be reminded, you will receive schedule reminders (push notifications and badges on your smartphone).
