To independently add a Symptoms/Mood to your care plan in Care at Home:

Regardless of being a patient in a Provider’s Directed Care Program or an independent consumer, the process for adding elements to your care plan are the same.

  1. Login to your Care at Home app.

  2. At the bottom of your home screen, you will see a few icons, including “Plan”. Tap on the Plan icon. (If you are in a Provider’s Directed Care Program, you will see elements of your provisioned Care Template listed. If you are a consumer, new to Care at Home, the screen may not have any entries.)

  3. Tap on the “+” sign at the top right of the Care Plan screen to open the Regimen screen.

  4. On the Regimen screen, an option under the “Add a Care Plan Regimen” header for “Symptoms/Mood” will be presented for you to independently build and add your own self-care elements to your care plan. (Remember, if you are a patient in a Provider’s Directed Care Program, your Provider will NOT BE NOTIFIED if your new care plan elements have any alert notifications associated to them.)

  5. Tap on the “Symptoms/Mood” panel to open the “Details” screen.

  6. Next tap on the arrow at the right of Description and select the symptom you wish to track.

  7. Next tap the arrow under the Symptom Alert Rule to set the severity level that you wish to establish as the threshold for an alert notification, then tap “Save”. (Note: Any caregivers that you have selected and have permissioned, and who have agreed to participate in your Circle of Care using Care at Home, can be notified any time you record a measure that meets or exceeds your self-established threshold.) IF YOU RECORD A MEASURE WHICH EXCEEDS A THRESHOLD FOR A SYMPTOM THAT YOU SET FOR YOUR OWN INDEPENDENT SELF-CARE, AND YOU PARTICIPATE IN A PROVIDER’S DIRECTED CARE PROGRAM, YOUR PROIVIDER WILL NOT BE NOTIFIED ABOUT THAT SYMPTOM WHICH YOU SET FOR YOUR OWN INDEPENDENT SELF-CARE.

  8. Next under the heading “When will you need to do this” tap the arrow on the right side of the panel to open the Day(s) screen. The Day(s) screen defaults to Daily for scheduling the recording of this symptom. If you want to modify, just tap on the check mark (uncheck) to remove that day from the scheduled reminder for that day. Then tap “Save”. Note, if you select “As Needed” Care at Home will not generate any Reminders.

  9. On the next panel, under the heading “At what times of the day”, tap the arrow at right of “Select time(s)” to choose the time of day when you wish to receive your reminder for this symptom. The tap “Save”.

  10. The next step is to select the dates on which you want to begin and end your scheduled recording of your symptom. Note that the beginning date defaults to the current date – you cannot establish a beginning date before the current date, but you can schedule a beginning date in the future. Also note that the end date defaults to “Never expires”. To set specific dates, tap on “Starts” and “Ends” to establish specific dates for scheduling this symptom. When completed, tap “Save”.

  11. When all selections have been made on the Details screen tap “Next” at the top right of the screen to go to the Summary screen

  12. On the Summary screen you can review your selections. If you want to make any modifications, tap on the arrow to the right of the symptom name in the top panel. This will reopen the Details screen where you can make changes. (follow the steps above to modify “Symptom Alert Rule”, “When you will need to do this”, “At what times of the day” and “How long will you need to do this”. Once you are satisfied with your selections, tap “Save” on the Summary screen.

  13. Next, on the updated Summary screen, you can set your Reminders and Alerts settings.

• With Reminders “on” (default check marked), you will receive notifications when this newly added Symptom measure is scheduled. Setting Reminders to “off” (unchecked) means you will not receive reminders when you are scheduled to record.
• With Alerts, you can let your Circle of Care (others who are supporting you using their own Care at Home app) know that you have missed a scheduled recording of this newly added Symptom.

13.  The final step is to tap “Save” on the Summary screen to complete the process.

Repeat this process for each Symptom measure you wish to establish and schedule for yourself. Each individual symptom is independently scheduled so that you can manage each uniquely according to your own needs.

  1. Tap the back arrow at the upper left of the screen to return to your home screen.

The next time you log in, your newly added Symptom can be accessed for recording by tapping the Check in icon in the upper right corner of your home screen listed under the heading of “(your name / Self”). If you set up a schedule for your self-managed symptoms, and selected to be reminded, you will receive schedule reminders (push notifications and badges).
