To independently add a Condition/Diagnosis to your care plan in Care at Home:
Regardless of being a patient or a consumer, the process for adding elements to your care plan is the same.

Login to your Care at Home app.

At the bottom of your home screen, you will see a few icons, including “Plan”. Tap on the Plan icon. (If you are in a Provider’s Directed Care Program, you will see elements of your provisioned Care Template listed. If you are a consumer, new to Care at Home, the screen will not have any entries. )

Tap on the “+” sign at the top right of the Care Plan screen to open the Regimen screen.

On the Regimen screen, an option for “Add Condition/Diagnosis” will be presented for you to independently build and add your own self-identified condition to your care plan. (Remember, if you are a patient in a Provider’s Directed Care Program, your Provider will NOT BE NOTIFIED if your new care plan elements have any alert notifications associated to them.)

Tap on the “Add Condition/Diagnosis” panel to open the “Condition/Diagnosis” screen.

Enter your information where it states “Search” and after a few seconds, based on the spelling of your entry, you will be presented with options from which to select the most appropriate choice. If you don’t see the appropriate condition/diagnosis, check your spelling or reenter. If there are still no results, re-enter the name to create a custom name or tap the back arrow to leave the field blank.

Tap on the panel of your choice then tap “Add” in the upper right corner of the screen to complete the process.
The next time you log in, your newly added Condition/Diagnosis will be listed under the Plan icon at the bottom of your home screen.
