In Care at Home language, a “Dependent” is a person who is supported by another person, a “Proxy”, who is enabled to support the Dependent by using the Proxy’s own Care at Home mobile app as a tool to support that other, Dependent person, on their health journey.

Initiate Dependent Account set up: An individual person who independently uses Care at Home can add a “Dependent” in Care at Home. To do so:

  1. Log in to your Care at Home app.

  2. From the home screen, immediately under the “Circle of Care” heading (be sure that the heading is “expanded”) tap on “Add Member”.

  3. On the pop-up window, tap “Create Dependent Account”.

  4. The next screen will be the Dependent account screen, which you will need to complete.

Dependent Name and Dependent Phone Number:

  1. Next, add your loved one’s (Dependent’s) name, and mobile phone number. (As Dependents do not participate using Care at Home, you may enter a land line number for a dependent if your loved one does not have a mobile number.)

  2. Add Photo or Meaningful Image: You may also wish to add an optional photo of your loved one (Dependent) or a meaningful image by tapping on “Edit image” under the “No Image” square at the top of the Dependent screen, which will open an option to “Take photo” or “Select from Gallery” (Android) / “Select from Photos” (iPhone) for images already captured on your smartphone. Follow the appropriate steps to take a photo or add the image you select.

  3. Date of Birth: Next, tap on the arrow at the right of the Date of Birth panel. Tap on the year at the top of the pop-up window to scroll through and select the Dependent’s year of birth, then tap the arrow on either side of the month to select the birth month, then tap the day of the month of birth. When that is completed, tap “OK” at the bottom of the window.

  4. Next, enter your loved one’s Zip Code.

  5. Select Time Zone: To complete the Dependent account profile, tap on the arrow at the right of the time zone panel to open a selection window. In the selection window, scroll to find the Dependent’s country, then select the appropriate time zone. After you select the time zone, you will be returned to the Dependent screen.

  6. In the upper right corner of the Dependent screen tap “Save”. Your Care at Home app will now be updated to include your loved ones’ new Dependent profile, which you will now see – and can access from the Circle of Care section of your home screen.

NOTE: An individual person who independently uses Care at Home and establishes a Dependent account, can directly engage as caregiver for that Dependent using Care at Home as a support tool. This is a voluntary action which does not carry with it any rights to, nor obligations of the Proxy or the Dependent. Care at Home provides this functionality strictly as a convenience for supporting a loved one. The Dependent being cared for does not participate in using the Care at Home mobile application in any way. The Care at Home account owner who is the Proxy of the Dependent, can invite other family and friends into the Circle of Care of the Dependent to contribute their own support, as Co-managers, caring for that Dependent.

Dependents in a Provider’s Care at Home Directed Care Program: A Dependent and Proxy relationship may also be established by a Provider for a Directed Care Program. In such circumstance, a patient authorizes the Provider to assign, as a Proxy, a person who agrees to participate on behalf of that patient under the Provider’s Directed Care Program. Once the Proxy accepts participation in the Program, the patient then becomes the Dependent of that Proxy, who can engage using Care at Home on behalf of the Dependent and communicate with the Provider’s care team as determined by the rules established by that Provider’s Directed Care Program. Ask your Dependent’s Provider if they offer Care at Home from Lifeguard.
