Deleting your independently created elements of your Care Plan

If you are a Patient in a Directed Care Program you have a Care Template provisioned to your Care at Home account from your Provider. If you have enlarged your care plan by adding additional elements (e.g., medications, supplements, and other health elements) to create a broader care plan, you can delete only those elements you have added, but cannot delete any elements of the Care Template provisioned to you by your Care at Home Provider.
Independent Care Plan for Consumers

If you are a Consumer and do not participate in a “Provider’s Directed Care Program” and you have independently created your own care plan elements (Conditions/Diagnoses, Medication/Supplements, Symptoms/mood, Vitals, and Wellness) for your own self-care using Care at Home from Lifeguard™, you can also delete those care plan elements at any time.
Managing regimen elements in your Care Plan
Whether you are a Patient in a Provider’s Directed Care Program or an independent consumer, the first two steps in the process for accessing your Care Plan to manage your regimen elements, which you independently use for your own self-care, is the same:

Login to your Care at Home app.

At the bottom of your home screen you will see a few icons, including “Plan”. Tap on the Plan icon to open the Care Plan Screen.

If you are a patient in a Provider’s Directed Care Program, you will see elements of your provisioned Care Template listed.

If you are a patient in a Provider’s Directed Care Program, and you have previously independently added your own care plan elements, you will see both the elements of your provisioned Care Template from your Care at Home Provider, and the elements of your independently created care plan listed.

If you are a consumer, who has previously created care plan elements, you will see those elements of your independently created care plan.

If you are a consumer who is new to Care at Home, the screen will not have any entries and there will be nothing to delete.

To DELETE independently created elements of your care plan: You may only delete the elements of your care plan that you have independently established for your own self-care.
After tapping on the Plan icon, tap on the name panel of any regimen listed on the Care Plan screen to open that element of your care plan.

For Vitals and Wellness: look at the bottom of the Summary screen. If “Created by” states “(Your Name) / Self”, you can delete this care plan element. If your Provider name is at the bottom of the Summary screen you may not delete this regimen.

Tap on “Edit” in the upper right of the screen.

At the bottom of the next screen, tap on “Delete Regimen”.

Next you will see a pop up window where you will have the option to “Confirm” the request to delete or “Cancel” the request to delete.

If you tap “Confirm”, the vital or wellness measure will be permanently deleted, and you will be returned to the Care Plan screen. If you want this particular vital or wellness measure added back to your care plan, you will need to initiate the process to add a new care plan element.

If you tap “Cancel”, you will be returned to the Summary screen.

For Symptom regimen, tap on the panel under the heading “Symptom” to open the measure you want to delete (the specific name will be under “Symptom” in the name panel). You may have more than one independently added symptom listed, in which case, each symptom will need to be deleted independently. (NOTE: If you have been provisioned a Care Template from a Care at Home Provider the Care Template name will also be listed with “Symptom Assessment” underneath that name. You may not delete this regimen or any element of the provisioned symptoms in the Care Template.)

At the bottom of the next screen, if “Created by” states “(Your Name) / Self”, you can delete this symptom measure.

Next, tap “Edit” at the top right of the Summary to open edit access to the select symptom.

At the bottom of the next screen, tap on “Delete Regimen”.

Next you will see a pop up window where you will have the option to “Confirm” the request to delete or “Cancel” the request to delete.

If you tap “Confirm”, the symptom will be permanently deleted, and you will be returned to the Care Plan screen. If you want this particular symptom of your care plan added back, you will need to initiate the process to add a new care plan element.

If you tap “Cancel”, you will be returned to the Summary screen.

Note: You will need to follow this process for every individual symptom that you wish to delete.

To delete Medications that you have independently added to your care plan, after tapping on the Plan icon:

Simply identify the medication you wish to delete and swipe on the name panel, right to left, to reveal a red “Delete” button.

Next you will see a pop up window where you will have the option to “Confirm” the request to delete or “Cancel” the request to delete.

If you tap “Confirm”, the medication will be permanently deleted, and you will be returned to the Care Plan screen. If you want this medication added back to your care plan, you will need to initiate the process to add a new care plan element..

If you tap “Cancel”, you will be returned to the Summary screen.
